What Birth Registration Questionnaire (9610) means

The Birth Registration Questionnaire (form 9610) is Vanuatu's birth registration form. It is completed for each family member, fully in English and in capital letters.

It contains own details (surname, name, place and date of birth, occupation, address), the father's and mother's details and the grandparents' names. If a parent has died, write DECEASED and the year; the parents' address is their own.

The birth registration certificate is issued from this form, and the passport from that certificate. So it is completed among the first.

A simple example

The 9610 forms for the whole family went first. The certificates were issued on time, and the passports were not delayed.

Key facts

01Form 9610

One form per family member

The applicant, spouse and every child in the application each need one. Without it, a person will not get a Vanuatu birth certificate.

02How to fill it in

In English, in capital letters

All fields are written in Latin script and in capitals only. Names follow the international passport.

03Relatives

Parents and grandparents

The form asks for father's and mother's details and the names of the grandparents. If a parent has died, write DECEASED and the year; parents' address is their own.

04What it is for

Birth certificate, then passport

The birth registration certificate is issued from this form, and the passport from the certificate. That is why it is filled in early.

Common mistakes

Writing in lower-case letters.

The form is completed in English, in capital letters only.

Leaving out the grandparents.

Grandparents' names are required. Find them out before you start.

Giving the applicant's address instead of the parents'.

The parents' section takes their own address. If a parent has died, write DECEASED and the year.
