What Form B (Vanuatu passport form) means

Form B (Passport Application Form) is the Vanuatu passport application, one per family member. It contains the applicant's details, the citizenship declaration (Dual citizenship box), height and eye colour.

Two signatures: top right inside the box under the photo, without touching the edges - it is transferred to the passport - and bottom left in the declaration with the date. Children from 12 sign in both places; under 12 the top box stays empty and a parent signs the declaration.

The Identity Witness and Witness Declaration blocks stay blank - they are completed on the island.

A simple example

A client signed only at the bottom. The form had to be re-signed and the passport was delayed by a week.

05 / Key facts

Key facts

01Form B

Vanuatu passport form, one per person

The Passport Application Form covers the applicant's details, a citizenship declaration with a Dual citizenship box, height and eye color. One is completed for each family member.

02Signature in the box

Top right, under the photo, not touching the edges

This signature is transferred into the passport. If it goes outside the box or touches the edges, the form has to be signed again.

03Declaration signature

Bottom left, with the date

The second required signature. A form signed only at the bottom is not accepted and comes back for re-signing.

04Children's signatures

From 12 they sign, under 12 the box stays empty

A child aged 12 or over signs in both places. Under 12, the top box is left empty and a parent signs the declaration.

Common mistakes

Signing only at the bottom, in the declaration

Form B needs two signatures: in the box at the top right and in the declaration at the bottom. Without the box signature the passport cannot be produced.

Letting the signature cross the box

Sign inside the box under the photo without touching the edges. If space is tight, sign slightly smaller than usual.

Filling in the Identity Witness and Witness Declaration sections

Leave these sections blank: they are completed on the island. Any entry there means a new form.
