Part 1 is the due diligence form: personal data, passports, criminal record certificates. Part 2 is the full Commission file: FIU letter number, certificates, medical report, signatures. Check fields are completed only after the result.

02Part 1 of the form

03Part 2 of the form
The full file for the Citizenship Commission
FIU letter number, certificates, medical certificate, signatures. The fields about the check are completed only after its result.
04Who fills it in
We complete it, the client only signs
Each family member receives a completed form and a "where to sign" guide with marks, so signatures go in the right places the first time.
Common mistakes
Sending the client a blank form to fill in themselves
The agent completes the form from the client's documents. The client receives a finished version and only signs.
Completing Part 2 before the FIU letter arrives
Part 2 is completed after the check result, because it includes the FIU letter number. Without it the form is incomplete.
Signing in the wrong places
Sign following the marked guide we send for each person. A wrong signature means re-signing and a delay.
